The five-part CMR form is an international road consignment note, with each set consisting of five carbonless sheets. The forms are translated into Polish, English and German.
Each set includes one red copy for the sender, one blue copy for the recipient, one green copy for the carrier and two black copies for general use. The pads are glued along the top of the short edge.
